Abstract

We report a case of twin pregnancy in HIV positive mother, in the year of 2003, with no use of antiretroviral drugs during pregnancy, in which vertical transmission occurred only to second twin. Objective: To analyze the factors that have influence over mother-child transmission of HIV and to review the available literature on this subject. Comments: According to most of the publications on the subject, delivery route and birth order showed themselves as determinant factors to discordant vertical transmission between twins. However, in our report, we have found the presence of receptors to HIV on cells of the placenta and/or the fetus as the determinant factor of the infection. Twin pregnancies allow a more solid evaluation on the influence of each factor involved in vertical transmission of the HIV, therefore being a very important subject for studies.
